Note: Oct 7th, both the wording below as well as some of the hard coded text used around the CCC compliant waiver system has changed slightly so clubs should update their waivers to the revised wording below as soon as possible.

Cross Country Canada has undergone an extensive review of it's waiver requirements with it's insurance company and has updated the wording required for CCC clubs use on their membership forms.  The main CCC Compliant waiver functionality regarding matching the signer name with the credit card name remains unchanged and you can read more about that here:  http://www.zone4.ca/help/Zone4.htm#Zone4_Club_Registration_System/Club_Waivers.htm The only thing that has changes is the wording of the waiver that you place in the CCC Complant waiver text area.  Below, is a copy of the required waiver text in both English and French and you can copy and paste that into your waiver area.  Be sure the edit it to use your own Club Name and Division on the top line.

This Club Waiver must be completed when applying for Club Membership

IN CONSIDERATION OF Cross Country Canada (hereinafter called CCC), "My Division" (hereinafter called the Division) and "My Ski Club" (hereinafter called the Club) accepting my application for membership in the Club and thereby allowing (subject to certain limitations and conditions such as applying for a valid CCC race license) my participation at events, competitions and activities run or sanctioned by one or more of CCC the Division or the Club I agree as follows:

Part 1 - WAIVER AND RELEASE  - FOR ADULT REGISTRANTS ONLY
As an adult (i.e., over the age of majority in my home province), I hereby for myself, my heirs, executors, administrators and assigns, waive any and all claims that I have or may in the future have against CCC the Division and the Club their executive directors, servants, agents, sponsors, supporters, employees, contractors or volunteers (all of whom are hereafter the Releasees) and to release the Releasees from any and all liability for any loss, damage, expense or injury including death that I may suffer or my next of kin may suffer as a result of my participation in any event, competition or activity run or sanctioned by one or more of the Releasees (hereafter the Event), due to any cause whatsoever, including negligence, breach of contract, or breach of any statutory or other duty of care owed on the part of the Releasees and further including failure on the part of the Releasees to safeguard or protect me from the risks, dangers and hazards associated with the Event as set out below in Part Two.
I hold harmless and indemnify the Releasees from any and all liability for any property damage or personal injury to any third party resulting from my participation in the Event.
I further waive and release any and all recourse which I may now have or may in the future have resulting from any decisions of the Releasees in association with the Event, including any claims I might have in connection with any cancellation or rescheduling of the Event for whatever reason.
I have read and understood this Waiver and Release prior to signing it. I am aware that I am an adult and that by signing this Waiver and Release I am waiving certain legal rights that I or my heirs or next of kin may have against the Releasees.

Part Two - ACKNOWLEDGEMENTS - FOR ALL REGISTRANTS
Risks and Dangers: I acknowledge that skiing and participating in any event, competition or activity run or sanctioned by one or more of the CCC the Division and the Club (hereafter the Event) involves risks and dangers including, but not limited to: accidents that occur traveling to the Event; encounters with other skiers, obstacles (man made or natural) or wildlife; steep and varied terrain, uneven snow conditions; falls or collisions; falling snow or ice; adverse weather conditions, frostbite; exhaustion. I freely accept and fully assume all risks, dangers and hazards associated with my participation in the Event and the possibility of personal injury, death, property damage or loss resulting therefrom.
Health: I declare that my physical condition, to the best of my knowledge, is adequate to participate safely in the Event and that no physician or other medically qualified individual has advised me against participating in the Event. I further acknowledge and agree that it is my choice whether I will obtain a physical examination prior to participating in the Event, and I hereby acknowledge and agree to assume all risks associated with not obtaining such examination, or if I do obtain an examination and am instructed not to participate in the Event, I hereby assume all risks associated with my participation.
Image Use: I authorize and consent to the publication in any format or media by CCC the Division and the Club of any materials containing my name, likeness or image in association with my participation in the Event, and I release to CCC the Division and the Club and all persons acting under authority or license from them, from any claims I might have due to the initial or authorized subsequent publication of such material.
Follow Rules: I hereby agree to abide by the Rules and Regulations imposed by CCC the Division and the Club in association with the Event, and to follow the instructions of the officials during the Event.

La personne qui fait une demande d’adhésion à un club doit remplir le présent formulaire au moment de son adhésion.

En contrepartie de mon acceptation comme membre du club "nom du club"  (nom du club, désigné dans le texte par le club) par le club, par la division "nom de la division" (nom de la division, désignée dans le texte par la division) et par Ski de fond Canada (désigné dans le texte par SFC), et considérant que cette adhésion me permet (moyennant certaines exceptions et certaines conditions comme l’obligation de détenir une licence de compétition valide de SFC) de participer aux manifestations, épreuves et activités de ski organisées par une ou plusieurs entités comprenant SFC, la division et le club, j’accepte de me conformer aux conditions suivantes. 

Première partie : RENONCIATION aux recours et EXONÉRATION de responsabilité (ADULTES SEULEMENT)
Je soussigné, déclare en mon nom propre, et au nom de mes héritiers, des liquidateurs de ma succession, de mes administrateurs et ayants droit, renoncer de façon irrévocable à toute prétention, actuelle ou future, à l’encontre de SFC, la division et le club, ainsi que leurs directeurs généraux, leurs représentants, agents, commanditaires, partisans, employés, entrepreneurs ou bénévoles (l’ensemble de ces-derniers étant considérés comme «les parties exonérées»), et je déclare également décharger les parties exonérées de toute responsabilité à la suite d’une per-te, dommage ou blessure, incluant le décès, dont moi-même ou un membre de ma famille pourraient être victime, en raison de ma participation à une manifestation sportive, une épreuve compétitive ou une activité (désignée dans le texte par «l’événement» ) organisée par l’une des parties exonérées, seule ou conjointement, et qui pourraient avoir été causées notamment par négligence, violation de contrat, manquement à une responsabilité légale ou à un devoir de diligence incombant aux parties exonérées et comprenant également tout défaut de la part des parties exonérées d’assurer ma sauvegarde et ma protection face aux risques, dangers et obstacles inhérents à l’événement et décrits dans la deuxième partie ci-dessous.
Je m’engage à indemniser les parties exonérées et à les tenir non responsables de toute blessure ou dommage matériel occasionné à un tiers, en raison de ma participation à un événement.
Je renonce également à toute action ou poursuite que je pourrais intenter, actuellement ou plus tard, concernant une décision prise par les parties exonérées en relation avec l’événement, ou concernant l’annulation ou le report de l’événement pour quelque motif que ce soit.
Je déclare avoir lu et compris ce formulaire de renonciation et d’exonération avant d’y apposer ma signature. J’ai atteint l’âge adulte et je reconnais, qu’en signant le présent formulaire, je renonce à certains droits et à certains recours que moi-même ou les membres de ma famille pourraient intenter à l’encontre des parties exonérées.


Deuxième partie : déclarations de consentement (À REMPLIR PAR CHAQUE PERSONNE)
Risques et dangers : je reconnais que la pratique du ski et la participation à un événement compétitif ou à une activité organisée par SFC, la division ou le club, seul ou conjointement, comportent certains risques et dangers, dont notamment : un accident au cours du déplacement pour se rendre au lieu de l’événement; la collision avec d’autres skieurs; la présence d’obstacles artificiels ou naturels ou d’animaux sauvages; l’utilisation d’un terrain abrupt ou accidenté et des conditions d’enneigement inégales; des chutes ou des collisions; des chutes de neige ou de glace; des conditions météorologiques défavorables; des risques d’engelures ou d’épuisement. Je reconnais que ces risques et dangers sont inhérents à ma participation à cet événement et j’assume librement et entièrement toute conséquence pouvant en découler notamment, blessure, décès, perte ou dommage matériel.
État de santé : au meilleur de ma connaissance, je déclare avoir une condition physique adéquate pour participer à l’événement en sécurité et je déclare qu’aucun médecin ou professionnel qualifié ne m’a déconseillé de participer à cet événement. De plus, je reconnais et accepte qu’il revient à moi seul de décider si je dois me soumettre à un examen médical avant de participer à l’événement. Par la présente, je reconnais et accepte les risques découlant de ma décision de ne pas me soumettre à un examen médical, ou découlant de mon choix de participer à l’épreuve malgré un avis de contre-indication, et j’accepte les risques associés à ma participation dans ce cas.
Utilisation de mon image personnelle : je consens à ce que SFC, la division et le club puissent utiliser dans toute forme de publicité des documents où figurent mon nom ou mon image en rapport avec ma participation à l’événement et je renonce à toute réclamation que je pourrais intenter envers SFC, la division et le club ainsi que toute personne agissant pour le compte de ceux-ci, du fait de la publication actuelle ou ultérieure dudit matériel publicitaire.
Acceptation des règlements : j’accepte de me conformer aux règlements imposés par SFC, la division et le club pour l’événement, et j’accepte de suivre les directives données par les officiels au cours de l’événement.
