Foothills Wanderers Orienteering Club and Orienteering Canada Informed Consent and Assumption of Risk and Release of Liability, Waiver of Claims, and Indemnity Agreements

To be executed individually by each participant who is 18 years of age and older or a parent/guardian of each participant who is under 18 years of age. (Participants between 13-18 years of age must also read and sign the agreement with their parent/guardian. Parent/guardian of participants 12 years of age and under must read and sign the agreement.)

As a participant or parent/legal guardian of a minor participant in activities of the Foothills Wanderers Orienteering Club, you agree to be bound by the following legal agreement.

The agreement will be displayed on the next page. If you do not agree, you can close the page without submitting your agreement. If you submitted it and did not intend to provide consent to the following agreement, please notify the Club immediately by emailing president@orienteeringcalgary.ca

Note that our online waver must be submitted for each participant individually, even if you previously agreed to similar terms.

To submit waivers for multiple participants, click the "Add New Person" button after each waiver is completed.

A copy of this agreement will be sent to the email address below.

Important: once you have completed all waivers for your family members, you you must press the "Finish" button to submit your waiver(s).
