Please complete for each athlete.

Please note that enrolment for the Junior Development program, for ages 8 to 12, will be done through the Skills Development Program.  Registration and payment for this new program is available within the main club registration.

The purpose of this registration is to make sure that each athlete has been enrolled in a program being managed by coaches of Team Telemark.  You will not be able to or required to pay for team fees here.  In order to avoid large credit card processing fees, we have opted to deal with payments the old fashioned way;by cheque.
