Welcome to Dead Ox Trailblazers Membership Form

Please choose your membership type and then proceed to fill in the individual member form for each member of your family.

Membership Types

Family is $25 plus $38 (goes to CCSAM) and consists of 2 adults of the same household and their dependant children under the age of 18 yrs (as of Dec 31st of the current registration year)

OR

Individual Pricing can be chosen on the following pages and is as follows:

  1. Adult 18 yrs & older - $8 plus $18 (goes to CCSAM)
  2. Youth 12 - 18 years old - $7 plus $18 (goes to CCSAM)
  3. Child 6 - 12 years old - $7 plus $18 (goes to CCSAM)
  4. Child 5 years & under - $0 plus $18 (club pays $18 to CCSAM)
