Team Captain Information

The information requested below is for the Team Captain only.  The Team Captain is responsible for conveying all information to his or her team members.

You MUST download the following waiver and print enough copies so that each team member can complete and sign one in order to be eligible to play.  Bring the completed waivers with you on game day to be submitted at the sign-in table: PlayerWaiverForm.doc \

Teams will consist of 6 players.  Games will be played as two ten minute halves of exciting 3 on 3 action (plus goalie).

*Please note that every team must have at least one female member

Team check-in begins at 7:00 am on September 24 with game play beginning at 8:00 am.  All teams MUST check in and wear the awarded conference sponsors' t-shirts.
