(2) Read and respond to the Privacy Statement, Release Information, and Club Waiver.

(3) Select "Continue" at the bottom of this form and then after reading the summary select "Add Person" to proceed to the next step to fill in an individual data form for each member of your family.

(4) Continue to click "Add Person" until all family members are entered. All members must be registered for insurance purposes.

(5) Proceed to checkout choose payment by credit card or cheque/cash (for cash/cheque print waiver).
