Trent Severn Waterway Brigade - Final Crew Registration and Payment

This page should be completed by the Crew Leader for one big canoe, if you are responsible for more than one canoe please register each canoe separately. Please proceed to fill in the leader, canoe section, the name for each member of your crew, their CVBS Membership #/Receipt# , and emergency contact person with best phone # . If you have more than 12 crew members, please submit their individual CVBS #, and emergency contact person with phone# by email to B. Milne <brucehmilne@gmail.com>.

Please note this is a "one-time" form and is your final registration. You will not be able to make any changes to this form after submission until on-site registration check-in at Discovery Harbor on July 27. You must have all your crew information as noted above "at hand" to complete this form and payment.

Brigade Final Registration fee is $2500 per crew. Ideally crews should have 8 to 10 members, if paddling a standard six seat Centennial North Canoe. Crews greater than (>)10 members are allowed at $150 for each member over 10.
