RELEASE AND WAIVER
Event:  BAM October Training Camp

Date of Event: Sat-Sun Oct 13/14th, 2012


In consideration of the Biathlon Association of Manitoba, Inc. accepting my entry into the above named activity,
I _____________________________________________________ hereby for myself, my heirs, executor, administrators and
assigns, release and forever discharge the Biathlon Association of Manitoba, Inc., it's servants, agents, sponsors, or employees
from all claims, demands, damages, actions or causes of actions, arising out of or in consequence of any loss, injury or damage
to my person or property incurred while attending at or participating in the above named activity, notwithstanding any such
loss, injury or damage may arise by reason of the negligence of the Biathlon Association of Manitoba, Inc. its servants, agents,
sponsors or employees.

Without limiting the generality of the foregoing, I further release any and all recourses which I may now or
hereinafter have resulting from any decisions of the Biathlon Association of Manitoba, Inc.


RELEASE AND INDEMNIFICATION

In consideration of the Biathlon Association of Manitoba, Inc. accepting the entry of the above-named participant in the
above-named activity, I _____________________________________________ Parent/Guardian of the said participant hereby
for myself, my heirs, executors, administrators and assigns, agree to release and to indemnify and save harmless the Biathlon
Association of Manitoba, Inc., its servants, agents, sponsors or employees from any and all claims or demands whatsoever
which might be made against the Biathlon Association of Manitoba, Inc. arising out of or in consequence of the above-named's
participation in the above named activity, notwithstanding that such claims or demands may arise out of the negligence of the
Biathlon Association of Manitoba, Inc., it's servants, agents, sponsors or employees.
