This Final Registration Form should be completed by the Crew Leader for one big canoe. If you are responsible for more than one canoe please register each canoe separately.

Before you begin filling out this form, please note:

• You will not be able to make any changes to the form after submission.
• You will need the names of your crew members; their CVBS Membership numbers; their phone numbers; and their emergency contact information.

Any changes require after submission may only be made on-site at the Registration Check-in, Battery Provincial Park campground, St. Peters, on August 11.

REGISTRATION DEADLINE:

• Final Registration Form and payment of remaining fees is July 11 .

PAYMENT SCHEDULE:

For crews of up to ten (paddlers and/or support crew), the fee is $3000 .

• Initial deposit upon registration = $300
• Remaining fees upon final registration = $2700

For crews of more than ten , (paddlers and/or support crew), the fee is $100 per additional person.  All participants receive camping, swag, cultural presentations, and a minimum of two suppers.

If necessary, crews may withdraw and be refunded as follows.

• Initial registration fee of $300 will be retained for all cancelled registrations.
• Cancellation received on or before July 31 , then $1,500 will be retained.
• Cancellation received after July 31 , then full registration fee will be retained.
