1. BEFORE completing this form - Complete a SEPARATE Waiver for each Passholder Sign Digital Waiver Here
2. THEN START THE SEASON PASS FORM
3. Have a photo ready to upload before starting
4. Fill in all information (name, address, etc.)
5. For more than one Pass - see end of application "Add Another Person"

Payment can be made by credit card, e-transfer or cheque

• To pay by E-transfer or cheque - check "Paid by cheque" at bottom of form
• Send E-transfer to: skiphoenix@gmail.com
• Please use "phoenix" for the security answer on E-transfers
