October 25th (U14) and 26th (U16 & FIS)

Time : 8:30am to 4:30pm

Please note: Time slots are 2 hours long and will be assigned and communicated on Thursday, October 23rd.

A NEW WAIVER & FORMS MUST BE COMPLETED BY THE PARENT/GUARDIAN FOR EACH INDIVIDUAL REGISTRANT PRIOR TO THE TESTING DATE:

To finalize your registration, you must complete three mandatory forms found at the link below.

-Consent for Testing

-Get Active Questionnaire

Step-by-Step Instructions

1. Click the link above. You will see a list of green boxes—these are the individual forms.
2. Click on the first green box to complete one of the forms (e.g., CSI Alberta Registration).
3. IMPORTANT: When you submit the first form, it will NOT automatically take you to the next one.
4. After submitting, you must return to the original link (pasted above) and click the next green box to complete the second form (Consent for Testing).
5. Repeat this process one more time to complete the third and final form (Get Active Questionnaire).

Please ensure you have clicked on and submitted all three green boxes to fully complete your registration requirements.

If you have multiple athletes registering, you will have to repeat the process above for each athlete.
