Please note: Time slots are 4 hours long and will be assigned and communicated on Thursday, October 22nd.

A NEW WAIVER & FORMS MUST BE COMPLETED FOR EACH INDIVIDUAL REGISTRANT PRIOR TO THE TESTING DATE. Where a participant is under the age of 18, all required forms must be completed and signed by the participant’s legal parent* or guardian.

Please note that there are three distinct forms that need to be completed for this camp. This may require the participant to revisit the link three times to choose and complete the required forms:

• CSI Alberta Registration
• Consent for Testing
• Get Active Questionnaire

All required forms must be completed and submitted 5 days prior (OCTOBER 19th) to the first scheduled assessment date (OCTOBER 24th).

Step-by-Step Instructions

1. Click the link above. You will see a list of red boxes—these are the individual forms.
2. Click on the first red box to complete one of the forms (e.g. CSI Alberta Registration).
3. IMPORTANT: When you submit the first form, it will NOT automatically take you to the next one.
4. After submitting, you must return to the original link (pasted above) and click the next red box to complete the second form (Consent for Testing).
5. Repeat this process one more time to complete the third and final form (Get Active Questionnaire).

Please ensure you have clicked on and submitted all three red boxes to fully complete your registration requirements.

If you have multiple athletes registering, you will have to repeat the process above for each athlete.

* Fine Print

For registration-only events, completed forms are required a minimum of five (5) business days prior to the assessment date. For events that include physical assessments, where a CSEP Get Active Questionnaire(GAQ) and Consent Form are required, completed forms are required a minimum of ten (10) business days prior to the assessment date. This extended timeline allows our staff to review all submissions and, in the event a positive response is recorded on the GAQ, to ensure sufficient time for physician clearance to be obtained.

Where a participant is under the age of 18, all required forms must be completed and signed by the participant’s legal parent or guardian. No other individual, including but not limited to coaches, instructors, or other supervising adults, is authorized to sign forms on behalf of a minor unless they have documented legal authority to do so.

CSI Alberta may request proof of identification, legal guardianship, or other documentation confirming the individual’s authority to provide consent or sign on behalf of the minor. Where appropriate documentation cannot be provided, services may be delayed until the required authorization is confirmed.

Walk-on participation is permitted provided that all required forms are completed and signed on-site prior to the assessment. For minor participants, a legal parent or guardian must be present to complete and sign all forms. In any case where a positive response is recorded on the CSEP GAQ, whether submitted in advance or completed on-site, the participant will not be permitted to participate until physician clearance has been obtained.

Participants who have not submitted all required forms within the stated timelines, or who are unable to satisfy the requirements outlined above at the time of the event, will NOT be permitted to participate in assessments.
